Certified copied of Missouri birth certificates from 1920 to present are available at a cost of $15.00 per copy.  Forms may be obtained at The Wayne County Health Center.

Birth Certified are needed for entrance to school, voter's registration, obtaining a driver's license, marriage license, passport, veterans benefits, social services and social security benefits and many other purposes.
Who May Recevie Copies of Birth Certificates?

*Registrant (the person whose certificate was filed)
*A member of his/her immediate family which includes those family members and in-laws in          the direct line of descent up to, but
not including cousins.
Wife
Husband
Son
Daughter
Mother
Father
Brother
Sister
Grandmother, Grandfather, Grandchildren
Aunt, Uncle, Niece, Nephew

*A guardian may receive a copy of the birth certificate of a child who is under his/her care
and custody by showing guardianship papers.
*Foster parents may receive a copy of a birth certificate of a child who is under their care and
custody upon furnishing a copy of their custody papers.
*A step-parent may receive a copy of a birth record by stating relationship.
*An attorney acting on behalf of  the registrant/or his or her family.
*A physician acting on behalf of the registrant/or his or her family.
*Authorized agent acting on behalf of the registrant/or his or her family.  An authorized agent
shall produce a signed statement by the registrant or a member of his/her family authorizing
the release of a record to the applicant or a notarized statement from family.
*Others may demonstrate a direct and tangible interest when information is needed for
determination or protection of personal or property rights.
